Under the Radiation Protection and Control (Ionising Radiation) Regulations 2000, all radiation incidents and accidents must be investigated and reported to the Radiation Protection Branch. This includes the loss or theft of a radioactive substance or radiation apparatus.

The ARPANSA’s Incident Form (118.8 KB DOC) is to be completed for a radiation accident involving radiation exposure from a radioactive substance or radiation apparatus. The incident form can be obtained by contacting the Radiation Protection Branch on (08) 8463 7825 or email.

The completed incident report will be reviewed by the EPA prior to forwarding onto ARPANSA for inclusion in the Australian Radiation Incident Register (ARIR). You may be aware that the ARIR maintains a database of radiation incident reports compiled from reports provided by the Commonwealth, State and Territory Radiation Protection Authorities.

Please forward the completed form with any supporting attachments to EPA Radiation Protection Branch: GPO Box 2607 Adelaide 5001 or email.
